Long Term Placement

An admission form and agreement must be completed and signed by the resident or sponsor prior to admission.

Relief Care Bed

The Department of Social Development sponsors a relief care bed in the Manor.  Families wishing to use this service may contact the Director of Nursing or Administrator.

The daily rate is established by the Department of Social Development and approved by the Board of Directors of the Manor.  It is the responsibility of the resident’s family or his/her sponsor to see that the resident’s account is kept current.

Upon admission the resident’s family will be asked to designate a sponsor. The sponsor is an individual who is the contact person and will administer the resident's funds to cover complete payment of nursing care fees and incidental expenses (e.g. hair care). The sponsor has responsibility and gives direction for resident’s belongings. The sponsor should be the person designated as Power of Attorney. A copy of the Power of Attorney will be requested by the Business Office upon admission. It is important that the sponsor keep the nursing home informed of change of address and/or telephone number.
